How to Complete The Annual Parental Consent Form

Each year, parents are required to complete the annual parental consent form. This must be completed before you can register any child into camps.

Step-by-step instructions

Step 1: Log in to your Family Account.

Step 2: Once logged in, select the My Forms tab from your account profile.

Step 3: Select 2026 Participant Information and Parent/Guardian Consent Form by clicking the button that says More Information.

Step 4: On the next page, select the Apply Now button.

Step 5: Select the Start button next to the child's name who you are filling out the form

Step 6: On the next page click Start

Step 7: Fill out the parental consent forms

Step 8: Click Submit Application to Finish
